Housing Stabilization Services (HSS)

It is a Medical Assistance (Medicaid) benefit to help people with limited abilities, disabilities, and seniors plan, find, and keep housing in the community. HSS are state plan (not waiver) Home and Community-Based Services (HCBS).

Minnesota supports people with disabilities, to live, work, and play in communities of their choice. The purpose of Housing Stabilization Services is to:

  • Support an individual’s transition into housing
  • Increase long-term stability in housing in the community, and
  • Avoid future periods of homelessness or institutionalization

HSS is comprised of three individualized, person-centered services:

DEVELOPMENT

Housing Consultation - People who do not have waiver or targeted case management or a MSHO/MSC+ care coordinator, but need Housing Stabilization Services, can complete the person-centered planning needed through an enrolled housing consultant.

PLAN & FIND

Housing Transition - Service that assist a person to plan for, find and move to a home in the community.

KEEP

Housing Sustaining - Service that supports a person to maintain living in their home in the community.

Relocation Service Coordination Targeted Case Management (RSC–TCM)

This is a Medical Assistance (MA) reimbursed case management service to help people residing in eligible institutions gain access to needed medical, social, educational, and other services and supports necessary to move from the institution to a community-based setting.

The Department of Human Services supports Relocation Service Coordination for the purpose of helping members gain access to needed services and supports if they choose to move from an institution to the community by the:

  • Development and implementation of a relocation plan
  • Coordination and monitoring of the implementation of the plan
  • Communication with the person, legal representative (if applicable) and informal supports
  • Communication with service providers
  • Coordination of referrals and assistance to access services and housing
  • Coordination of efforts with the discharge planner at the institution
  • Travel to conduct a visit with the person and others identified as necessary to develop and implement the goals of the relocation plan
  • Updates to the relocation plan when necessary
  • Completion and maintenance of documentation in case notes that supports and verifiesRelocation Service Coordination assistance to access needed services, including travel to visit a member to develop or implement the goals of the written plan.
  • Coordinate, monitor and support overall service delivery and advocacy as needed to ensure quality of services, appropriateness and continued need
  • Coordination with the facility discharge planner in the 180-day period before the member's discharge
  • Documentation that supports and verifies the activities
  • Routine contact or communication with:
  • Legal representative
  • Primary caregiver
  • Member
  • Member’s family members
  • Service provider(s) or other persons identified as necessary to the development or implementation of the goals of the written plan
  • Substitute care provider

HOPE MOVES

Moving can be a challenge and services can be expensive. Hope Outreach wants to partner with the community and try to meet the person where they are with affordability to access the essential supports to transition in the community. Hope has a dedicated team of courteous personnel who are willing to work to the individual’s needs. With HOPE the transition process is accomplished with care and professionalism. We can assist with providing the necessary supplies to make the transition as comfortable and seamless, as possible.

This is a service Hope Outreach provides to the public. Being publicly commissioned as a "ministerial" official, our duty is to screen the signers of important documents for their identity, one’s willingness to sign without duress or intimidation, and their awareness of the contents of the document or transaction.

As official representatives of the state, we will certify the proper execution of many of the life-changing documents of private citizens — whether those diverse transactions convey real estate, grant powers of attorney, establish a prenuptial agreement, or perform the multitude of other activities that enable our civil society to function.
